For the applicant selected for tentative placement into the Surgical Technology Department, the following will occur:

The applicant will receive a letter of notification and health forms from the Department Chair of the program.

The applicant must submit to the Department Chair, within 1 week of the date of the letter from the Department Chair, a written acceptance letter of placement. This can be mailed to the Department Chair, emailed, or faxed to 903.510.2931.

The applicant will be required to return all health forms: physician’s examination; a full release, signed by the physician, to perform patient care; immunization records; and Hepatitis B inoculation statement of intent. There is a deadline for returning these forms. Failure to submit all required health forms by the stated date in the letter of notification would result in forfeiture of placement.

All accepted students must maintain a grade of “C” (75%) or better in all required SRGT courses. A final grade of “F” or “WF” constitutes failure to pass. Reapplication to the program each year will require all new application forms and transcripts. Students dismissed from the program who wish to reapply must make an appointment with the department chair. Re-admittance will depend on reason for dismissal.
